New Return from Leave of Absence form

All PACFA registrants are entitled to apply for a leave of absence from the PACFA Register. Leave may be taken for a period of up to two years. Leave of absence will only be granted where a valid reason is provided, including parental or study leave, compassionate circumstances, extended travel, unemployment or illness.

Upon returning from leave, practitioners are required to provide evidence of current indemnity insurance, criminal history check, working with children clearance (if applicable).

PACFA’s new Return from Leave of Absence form allows returning members to conveniently upload all necessary documentation on a single page. This streamlined approach not only simplifies the return process but also ensures a smoother transition for practitioners resuming their professional roles after a temporary break.
