Clinical/Academic and Provisional Application Forms

To apply in these categories there is a non-refundable application fee. For details see the Schedule of Fees. The annual membership fee is payable upon approval of your application.

Before applying for Provisional or Clinical Membership, you will need to collate the following documents: 

Applicants who have completed a PACFA accredited training course will need to pay the relevant application fee, and upload the following information: 

  • certified copy of the academic transcript and completion certificate for your counselling/psychotherapy qualifications; and
  • If you are not opting for PACFA Master Insurance you will need to upload:
    • a copy of your personal indemnity insurance or
    • a letter from your employer to confirm that you are covered by insurance for your counselling/psychotherapy work
  • If applying for Clinical membership, please upload signed logs for 75 hours of supervision and 750 client hours, undertaken over 2 years since training was completed, using the Supervision and Client Contact Verification form.
  • If applying as an Educator - A certified copy of your academic transcript and completion certificate for your Educator qualifications, and evidence you have been employed in an educator role with a 0.5 workload minimum, over the past 2 years.

Applicants who have not completed a PACFA accredited training course will need to supply the documents above, plus; 

  • Evidence of 40 client contact hours and 10 hours of related supervision undertaken during training/placement;
  • Evidence that the course meeting the PACFA Training Standards ( - such as a course guidebook, unit outlines, syllabus etc; and
  • Pay the additional non-Accredited application fee. 

Student and Affiliate Membership Forms

Join the PACFA Register as a Member Association member

Practising members of PACFA Member Associations who meet the PACFA Register Requirements are eligible to apply to PACFA for listing on the PACFA Register.

To list on the PACFA Register as a member of a PACFA Member Association you will need to take the following steps:

1. Select the most appropriate Member Association for your needs from the list of PACFA Member Associations

2. Review the registration requirements for Registered Practitioners to ensure that you meet the eligibility requirements.

3. Join your selected Member Association

4. Apply to PACFA to list on the PACFA Register, go to Join the Register for MA members

Transfer from PACFA registration to individual PACFA membership

Applications to transfer from PACFA registration to individual PACFA membership can be made by practitioners who are currently belong to a PACFA Member Association and who are listed on the PACFA Register.

As these members have already been assessed for PACFA registration, there is no need to go through the full PACFA application process again. Please complete the Transfer Application Form. There is an administrative fee as detailed in the Schedule of Fees.
